The BC Safety Authority performs inspections on new installations, existing elevating devices and major alterations. All building owners that hold an operating permit are obliged to hire a licensed contractor to maintain the device -- we will revoke the operating permit if you fail to comply.

New installations or major alterations
If you have a new installation or you’ve had a major alteration to an existing device, you must arrange an inspection before you put it into service.
To arrange the inspection and confirm you’ve completed your inspection preparation, please complete all of the following forms:
- Request for Acceptance Inspection Form 1148
- General Contractors or Owners Checklist Form 1222
- General Contractors or Owners Escalators/Movingwalks Checklist Form 1108
- Restricted Access & FEO Phase II Operation Test Declaration Form 1366
When we contact you with the date of your inspection you need to complete the relevant form for your device and be prepared to show it to the Safety Officer when they arrive for an inspection:
- Data Sheet Handicapped Stair Lifts Form 1210
- Data Sheet Handicapped Hydraulic Platform lifts Form 1211
- Elevator Data Sheet Checklist Form 1214
- Emergency Power and Fire Service Checklist Form 1212
- Escalator Data Sheet Checklist Form 1216
- Hydraulic Elevator Data Sheet Checklist Form 1213
Maintenance and testing
If you’re a licensed contractor, complete the following forms to confirm that you’ve completed the maintenance and testing requirements of your licence obligations. Submit by mail or in person to any BCSA office.
Building owners should be aware that we will inspect existing elevating devices to ensure you’ve arranged proper maintenance.
- Electric Elevator Brake Test Declaration Form 1208
- Electric Elevator Periodic Tests Declaration Form 1209
- Escalators/Moving walks Periodic Test Declaration Form 1109
- Handicapped Lift Periodic Tests Declaration Form 1110
- Hydraulic Elevators Periodic Safety Tests Declaration Form 1205
- Traction Sheave Brake Test Declaration - Electrical Elevators Form 1221
Non-Compliances
If your certificate of inspection identified a non-compliance you must resolve the issue and confirm you have completed the work by submitting the Elevating Devices Non-Compliance Completion Notification Form 1073 in person to the BCSA New Westminster office.
Failure to resolve the non-compliance will result in a new inspection fee or other enforcement actions. We may also decide to take the elevating device out of service.
If you would like to apply for exemption or a variance from the safety code requirements, please complete the Request for Variance - Elevating Form 1007 and submit by mail or in person to the BCSA Elevating Devices Safety Manager at the New Westminster office.
